Penn High School will be hosting the annual Sport’s Physicals with the doctors from St. Joseph Regional Medical Center.  This event will take place on Tuesday, May 19th in the Aux gym at Penn from around 3:15 pm – 5:00 pm and is only for high school students and incoming freshmen here at Penn.  This physical will cover students to participate in athletics for the 2015-2016 school year.  The cost for the physical is $25.   Penn encourage athletes to participate in this wonderful opportunity, so you don’t have to wait until this summer while everyone else is getting physicals.
The physical must be completed on the IHSAA Physical Form, which can be found on the IHSAA’s website, the Penn Athletics website, in the Athletic Office, and an attached copy is below.  Athletes and/or parents must complete the form, including all parent/student signatures, prior to attending.  The doctor will fill out their portion (page 2) during the physical.   Please have the form filled out ahead of time.  This helps the athletic staff from not having to hunt you down later and disrupting athletes practice time.
